Jewelry Care Membership — Service Request

This form is for active Jewelry Care Membership holders to request cleaning and inspection services.

Before mailing any jewelry, please complete the form below so I know what to expect and can confirm available care credits.

Please include the following in your message:

Your full name and email used for your membership

A brief description of each piece you plan to send

The total number of pieces

Any specific concerns (tarnish, wear, loose stones, etc.)

Once your request is reviewed, you’ll receive confirmation along with packing and shipping instructions.

Please do not mail jewelry without submitting a request and receiving approval first.
